Successfully passed a Board-approved, hands-on course in the functions and duties permitted to be performed by a Level III Dental Assistant. Must have completed a DA Level I and II program approved by the District of Columbia Dental Board to be eligible to take the DA Level III course that includes : Each course is $350 (a)The application of pit and fissure sealants; (b) Coronal polishing to remove stain and biofilm; and (c) Glucose testing Total $1,050
